Forms
Auxiliary National has developed standardized forms that are
universally used throughout the Auxiliary. Many of these forms may be
downloaded from
Nationals Forms
webpage. Some of the forms are designed to be completed electronically,
saved and printed. These forms and others may also be ordered through the
Auxiliary National Supply Center (ANSC) by the Flotilla Materials Officer.
Several of these forms are also designed for
on-line submission.
Auxiliarists should periodically check
Nationals Forms webpage for revised
forms as well as new additions to the electronic forms list. National is
currently posting revised forms on a quarterly basis. At minimum, you need
the Adobe Acrobat Reader software to open the form files. Download and
install the Adobe Acrobat Reader using the link provided
at the bottom of this page.
The following is a list of a few, frequently used forms that
are available on-line and the purpose of the form. Check the
Forms
webpage for a complete
listing. District guides for routing many of the forms can be found on the
Forms Routing Guides webpages which also
includes a direct link to other downloadable reports from National's Forms
archive files.

Change of Member Information - form
7028 -
report personal member information that will be entered on the AuxData system
i.e. change of address; new or changed e-mail address, telephone number;
emergency information; name change; etc.

Member Activity - form
7029 - used to report activity,
preparation and travel hours not reported on other forms i.e. activity,
preparation and travel hours for a Flotilla meeting; preparation and travel time
for Marine Dealer Visits; self-study, class, testing and travel time for
Auxiliary courses; time spent completing and mailing forms; etc.

DD-1351-2
completed for reimbursement of expenses following traveling i.e.
training at "C" school, District conference, etc. Form is self checking,

completed when an Auxiliarist would like to have a vessel they own approved as a
facility and/or offer it for use; inspected when the approved period expired;

used to secure approval for use of a marine radio as a land or mobile facility;
inspect the facility when the approved period expires.

Personal Watercraft (PWC) Inspection and Offer for Use

7008 - used to report a PWC vessel facility inspection and offer for use
as well as to report changes in the status of a facility.

Vehicle Facility Offer for Use - form

used to report a Vehicle facility inspection and offer for use as well as to
report changes in the status of a facility.

Coast Guard Districts have the prerogative to deviate from the
standard forms and incorporate information specific or unique to their
needs as well as develop forms of their own. The following are
